Assist with treatment plan for each assigned resident. Complete required documentation. Maintain ongoing communications with physicians concerning resident care. Ensure effective interactions with patients and families. Perform other duties as assigned. Requirements: Must have a valid LPN license in the state of Ohio. (2) years nursing experience (Long term care preferred).
