POSITION STANDARDS Graduate of an accredited school of nursing, current New Hampshire LPN/RN license. Must possess a high degree of interpersonal relation skills and capability of relating to a variety of people and personalities. Considerable initiative, judgment, and leadership skills are also required. Must have the demonstrated ability to direct and manage personnel. Must possess strong organizational and time-management skills. Must be willing to learn.

Current BCLS certififcation required within 30 days of hire.

POSITION PHYSICAL REQUIREMENTS Anything listed here requires a pre-employment physical by Employee Health to determine if the employee is capable of meeting the requirements. Physical Activity: Must be able to lift, carry, stand, bend, squat, crawl, reach, and kneel using good body mechanics for sustained periods of time. Upper Extremity: Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must be in good general health and demonstrate emotional stability in order to cope with the mental and emotional stress of the position. Push/Pull/Lift/Carry: Must be able to walk approximately four miles daily. Must be able to lift/pull up to 35 pounds. PART TWO: FUNCTIONAL RESPONSIBILITY

 

Position Objective Responsible for ensuring the general well-being of residents through the delivery of high quality services in a manner that is consistent with the philosophy of assisted living and that complies with the New Hampshire Nurse Practice Act and Resident Bill of Rights. He/she is responsible for the management and supervision of the resident associates in the absence of the Nurse Manager.

 

Performance Expectation Participates in conducting and maintaining resident's assessments using the state assessment tool for assisted living. Promote accuracy and participate in the process of monthly medication records. Plans and delegates work assignments during shift. Assists in conducting admission assessments and orients new residents to the facility ensuring satisfactory integration into the facility/community. Demonstrates an appreciation of the heritage, values, and wisdom of the residents and an understanding of the aging process. Assists in the investigation of resident and employee incidents, identifies safety hazards, and institutes corrective action in a timely manner. Shares in the responsibility for providing and maintaining an environment that is conducive to the safety and physical and emotional well-being of residents and personnel. Monitors and maintains records reflecting residents' physical and mental condition. Reports significant changes in a resident's condition to his or her physician. Documents medication supervised and administered and notes resident's response. Follows through on verbal medication change orders received from the physician. Documents resident incidents. Demonstrates ability to function as a team member in sharing responsibilities for the administration of resident services in cooperation with all members of the team. Assists in orientation of new staff. Provides ongoing training of staff in order to provide high quality service consistent with the philosophy of assisted living. Promotes and facilitates interactive problem solving with staff. Demonstrate basic knowledge regarding Lifeline and PHB system, initiate problem solving if needed. Performs other duties as assigned.
