The 1:1 School LPN/LVN identifies and treats healthdisorders among students in the school setting. The 1:1 School LPN/LVN providesinstruction for the student’s general wellbeing and disease prevention. TheSchool LPN/LVN monitors the student’s physical conditions in the school settingand refers the student to appropriate resources as needed.  The work of the School LPN/LVN is performedin compliance with his/her LPN/LVN scope of practice and according to stateeducation codes.

Patient Care:

Provide direct, one-on-one care to assigned patients, ensuring their comfort and well-being. Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and feeding.

Vital Signs Monitoring:

Regularly check and record patients’ vital signs, including temperature, blood pressure, pulse, and respiration rates. Report any significant changes or abnormalities to the supervising nurse or physician.

Medication Administration:

Administer prescribed medications and treatments accurately and on schedule. Monitor patients for any adverse reactions or side effects.

Patient Education:

Educate patients and their families about health conditions, treatment plans, and self-care techniques. Provide guidance on nutrition, exercise, and lifestyle modifications to support recovery.

Documentation:

Maintain accurate and detailed patient records, including care plans, progress notes, and medication logs. Ensure all documentation is completed in a timely manner and complies with regulatory standards.

Communication:

Communicate effectively with patients, families, and healthcare team members to coordinate care. Participate in team meetings and care conferences to discuss patient progress and care strategies.

Emergency Response:

Respond promptly to patient emergencies, providing first aid or CPR as needed. Follow established protocols for emergency situations and notify appropriate personnel.

Minimum Requirements:
