JOB SUMMARY

The Licensed Practical Nurse serves as a member of the healthcare team at UVA Health Northern VA & Culpeper by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with other team members to provide care that promotes optimal health. The Licensed Practical Nurse utilizes best scientific evidence and compassion to assist the care of the patient during their health journey. The Licensed Practical Nurse accepts accountability for provision of care, as part of the RN led team, in accordance with the current policies and procedures. Provides and documents patient care under the direction and supervision of the Assistant Nurse Manager/Nurse Manager.

Patient Care: Provides evaluation, and management of patients.  

Participates in the collection of patient data as needed.  Determines nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.   Identifies relevant resources based on necessity and availability. 

Communication: Communicates information to others responsible for or involved in the care of the patient. 

Participates in dialogue with the patient/chosen support system to identify the individualized care plan.  Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care.   Documents appropriate information in a timely manner. 

Teamwork: Participates as part of the interdisciplinary care team in planning individualized care.  

Collaborates with the registered nurse to consider ongoing modification of care plan based on patient’s needs, capabilities, and condition. 

Medical Knowledge: Demonstrates core knowledge of biomedical and clinical sciences and the application of this knowledge to patient care.  

Identifies nursing interventions and goals for the patient and reviews goals with the registered nurse.  Identifies opportunities for process improvement, evidence-based practice, and research.   Engages in self-reflection and develops personal goals to enhance knowledge, obtain professional certification, or pursue academic degree.  Integrates gained knowledge into practice.  

The incumbent may be asked to perform additional duties as assigned.

 

QUALIFICATIONS

Education: High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of a nursing program as approved by the State Board of Nursing.

Experience: 1 year of relevant experience preferred. 

Licensure: Applicable state LPN license required. Women & Children Services – BLS, ACLS (L&D), NRP, AWHONN Intermediate Fetal monitoring and/or NCC EFM Certification within one year. CPI certification within three months of hire required for Behavioral Health, Emergency Department, and Central Staffing and Scheduling Office (CSSO).

The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
