LPN-Post Transplant Kidney

The University of Minnesota Solid Organ Transplant Program has a long legacy of excellence in training doctors around the world in transplantation and in providing expert care to liver transplant patients. We have pioneered many innovative approaches to transplantation through the years. Today, more than 18,000 patients, including children, in the US are waiting for a new liver. As new surgical techniques and better immunosuppressant drugs are developed, more patients with liver disease have become eligible for a transplant; in some of these cases, the best option for transplantation is with a living donor. We have been performing this procedure since 1997. **Responsibilities Job Description** As a member of the care team, the Transplant LPN supports quality, patient and family centered care. LPN performs a variety of high standard functions within a care team to support the needs of the healthcare team and patients/support team. The LPN will assist the RN Transplant Coordinator in all activities related to patient care. LPN's will communicate closely with RN's & Transplant Coordinator to deliver a full continuum of care for the patient. Delivers care as delegated by the physician/authorized practitioner or RN, to assigned patient population, or individual patient. + Answers & assists with returning patient calls as advised by coordinator. + Records / transcribes for interdisciplinary committee meetings + Requests medical records needed for post-transplant care management + Notifies patient regarding required follow up for post-transplant cares, as delegated by the transplant coordinator + Enters lab orders and sends out lab letters as directed by the transplant nurse coordinator + Places electronic requisitions for specialized lab-testing, as instructed by the transplant nurse coordinator + Processes prescription refill calls as directed by the coordinator. + Manages incoming electronic and faxed medication refill requests to be signed by the transplant nurse coordinator + Works in conjunction with transplant schedulers, transplant administrative staff and coordinator. + Assists with preparation of vital forms for physician signatures required for post-transplant medications and cares + Assists with maintenance of patient electronic files + Assists with coordination of post-transplant care including scheduling routine visits and testing per post-transplant protocols and communication with patients. **Qualifications** **Required** + Graduate of an accredited post-secondary practical nursing program + Current Minnesota LPN License + Current AHA BLS certification. **Preferred** + Previous experience in ambulatory care setting **Additional Requirements:** High standard of creative prioritization skills and priority setting. Shown focus on customer/patient service with every interaction. Excellent public relations and communication skills. Work as an effective team member provide outstanding care. Ability to use appropriate computer applications. Ability to work independently, demonstrate critical thinking skills and engage in ongoing process improvement.
