LPN Scheduler - Home Care

Summary

IU Health Home Health provides our patients with comprehensive care delivered by a highly trained interdisciplinary team. Home Health Scheduling works with Referral Sources, MD’s, Home Care Case Managers for the delivery of care. The Home Care LPN Scheduler will monitor the IP referral for discharge and ensure start of care within the regulatory time frame for delivery of care. The Home Care LPN Scheduler will confirm provider to follow the patient during homecare services and be an intricate part of the care delivery set up.

Shift

Full Time Day Shift 40 Hours Per Week Monday - Friday 8:00 a.m. - 4:30 p.m. No On-Call Holiday rotation by volunteer Hybrid - with in person meetings and occasional need to come to the office

Requirements

Must have Home Care scheduling experience Requires graduation from a nationally accredited licensed practical nursing program. Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license. Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. Successful completion of orientation required. 1-2 years of relevant experience preferred.
