Summary:
Under the direction of the registered nurse, provides nursing care in accordance with departmental and hospital policies and procedures. The Licensed Practical Nurse (LPN) assists the RN with assessing, planning, implementing, and evaluating patient care. The LPN is responsible for documentation of interventions, outcomes, and patient/family education. They collaborate with physicians and other members of the interdisciplinary health care team to provide continuity of care. The LPN is responsible for the direction and assignment of nursing interventions that may be performed by others. Must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization.
The LPN assists in the nursing care and preparation of the surgical patient by performance of routine and delegated duties according to the policies and standards of the Center. Assembles appropriate supplies, drapes, instruments, equipment and sutures for the surgery procedure. Performs scrub duties in compliance with the principles of surgical asepsis and patient safety. Sets up and maintains sterile field and provides assistance to the operative team. Will rotate schedule between the operating room and the decontamination/sterile prep area. In decontamination/sterile prep area, will clean, sterilize, and wrap instruments according to policy and procedure. Responsible for checking supply inventory par levels and ordering supplies as needed.

2. Competent use of the nursing process to assess priorities, formulate, implement, evaluate and adapt the interdisciplinary plan of care to ensure quality of care is delivered.
Performs admission assessment in accordance with specific unit standards.Identifies and documents patient’s plan of care.Identifies and documents patient/family/significant other teaching needs upon admission.Identifies discharge planning needs of patient/family/significant other on admission.Reassessment of plan of care is done within 24 hours and ongoing, until outcomes are met or discharged.Assesses the patient/significant other's response to teaching plan to evaluate the effectiveness of the plan.Based on reassessment, the plan of care will remain the same or be modified to meet the patient/significant other's identified needs.Delegates and assigns appropriate patient care.Recognizes changes in patient status based on interpretation of assessments and carries out physician’s orders.
3. Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient in collaboration with RN.
Demonstrates knowledge of hospital policies and procedures for administering, verifying, and recording medications.Demonstrates knowledge of commonly used meds: action, dose, and side effects.Functions successfully in an emergency situation. Maintains professional composure and confidence during stressful situations.Identifies patient care problems and communicates them to RN.
4. Maintains up-to-date and accurate documentation of nursing care provided to ensure the integration of information for use by the healthcare team, to ensure quality care.
Each identified problem must be addressed every 24 hours.Nursing documentation is reflected in the patient medical record with an entry made at least every shift per standard and when reassessment of the patient is required.Nursing handoffs reflect the current plan for the patient and shift updates.
5. Performs all responsibilities/duties required by the Faith Regional Surgery Center as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes but is not limited to the age of the patient served.
Establishes priorities in organizing intra-operative care based on case type and needs of the patient and surgical team.Implements physician preference cards based on case type.Demonstrates operational knowledge of equipment and instrumentation.Keeps self-educated on new procedures and equipment.Clean and sterilize all instruments in accordance with policies and procedures.Maintains working status of instruments and equipment: Ascertains that broken and/or missing instruments are repaired or replaced; notifies Director of Nurses if loaner equipment is needed.Able to recognize emergencies and assist in corrective measures.Assists in maintaining a clean, conducive work environment.Assists in putting delivered laundry away.Respects and complies with decisions name by the Administrator/Facility Manager and the Director of Nurses; communicates with others in a direct, respectful, and effective manner; will treat customers with actions that exemplify high levels of customer service.
6. Assumes all other duties and responsibilities as necessary.
Demonstrates willingness to identify and/or assume activities relative to the developmental needs of the unit and the nursing department.Demonstrates dependability/flexibility in meeting scheduling needs of the unit and hospital.Demonstrates willingness to float to other units as needed.Participates in shared leadership.If in charge, provides for the safety of department employees.Accepts assignment as a coach for new staff.Accepts responsibility for mentoring healthcare students.
Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

EDUCATION:
Graduate of an accredited school of nursing required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Current Licensed Practical Nurse (LPN) license or temporary from the State of Nebraska required.
Basic Life Support (BLS) obtained within one full month from date of hire required.
