Summary of Job Duties:The Social Worker/Case manager will work on the Maternal Child Health Strategy under the direction of the Healthy Start Program Manager. This position provides direct services to clients and families within maternal child health programs. As a member of a health care team, the Social worker assists in developing and implementing an appropriate plan of care to meet client/family needs as well as education, support, and access to all available social services. Evening and weekend hours are required as needed.
Qualifications:

Minimum Qualifications

Bachelor’s degree in social work.Must have a current Arkansas Social Work License-LSW Valid AR drivers' license required with current automobile insurance and reliable car for moderate travel to LR occasionally and to surrounding communities (mileage reimbursed at the current state mileage reimbursement rate) if a state vehicle is not available. Occasional out-of-state travel.Must be able to work evenings and weekends as needed.Remote work allowed.

Knowledge, Skills & Abilities:

Ability to work under deadline pressure.Strong clinical judgment and assessment skills.Proficient in conducting assessments, formulating treatment plans, and providing evidence-based interventions.Knowledge of community resources and social services.Effective communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.Ability to work independently and collaboratively in a fast-paced environment.Cultural sensitivity and competence in working with diverse populations.Commitment to ethical practice and ethical standards of practice.

Preferred Qualifications:

Two (2) years of social work experience.Preferred counties: NWA or South Arkansas.

Key Responsibilities:

Promotes Screenings, consultation, and oversight of case management processes and procedures. Partners with community health workers and nurse educators to use screenings and motivational interviewing to assess current needs and set care plan goals for program participants.Monitors and evaluates clients’ progress and modifies care plans accordingly.Refer and connect clients to community services as appropriate.Develops and manages relationships with program participants and community stakeholders. Coordinates communication with community referral partners. Identifies barriers and utilizes problem-solving skills to address issues that arise regarding participant needs, ensuring all program objectives are met and data collection processes and procedures are followed. Performs quality checks for case management plan delivery.Review care plans for maternal, child, and fatherhood program participants to ensure goals align with identified needs.Supports continuous quality improvement within the programs and projects by addressing deviations as they arise. Coordinates training, referrals, or support services for improved case management services. May perform other duties as assigned.
