Job Title: Licensed Vocational Nurse (LVN) – Clinic Case Management

Job Description:

We are seeking an experienced Licensed Vocational Nurse (LVN) to join our clinic team. In this role, you will work closely with adult patients, focusing on documentation and case management. The ideal candidate will bring strong healthcare expertise and excellent organizational skills to support efficient patient care and clinic operations.

Responsibilities:

Conduct patient assessments and document medical historiesPerform case management and coordinate patient care plansHandle insurance verification and assist with triage processesMaintain accurate documentation in electronic medical record (EMR) systemsProvide exceptional customer service and patient supportUtilize medical terminology effectively for documentation and communicationEnsure compliance with healthcare regulations and clinic protocols

Required Skills & Qualifications:

LVN certification (Active and in good standing)Strong documentation and data entry skillsProficiency in EMR systems and medical terminologyExperience in case management and patient careExcellent customer service and communication skills

Experience Level: Intermediate

Pay and Benefits

The pay range for this position is $35.00 - $40.00/hr.
