Job Title: Licensed Vocational Nurse (LVN) – Clinic Case Management Location: South LA Job Type: Full -Time Clinic Setting Job Description: We are seeking an experienced Licensed Vocational Nurse (LVN) to join our clinic team. In this role, you will work closely with adult patients, focusing on documentation and case management. The ideal candidate will bring strong healthcare expertise and excellent organizational skills to support efficient patient care and clinic operations. Responsibilities: + Conduct patient assessments and document medical histories + Perform case management and coordinate patient care plans + Handle insurance verification and assist with triage processes + Maintain accurate documentation in electronic medical record (EMR) systems + Provide exceptional customer service and patient support + Utilize medical terminology effectively for documentation and communication + Ensure compliance with healthcare regulations and clinic protocols Required Skills & Qualifications: + LVN certification (Active and in good standing) + Strong documentation and data entry skills + Proficiency in EMR systems and medical terminology + Experience in case management and patient care + Excellent customer service and communication skills Experience Level: Intermediate If you are passionate about patient care and thrive in a clinic setting, we encourage you to apply and become part of our dedicated team!
