Our Company Apreva Hospice Overview The Hospice Triage LVN is responsible for assessing, organizing, and directing hospice care within the office setting, with occasional visits to patients as needed. This role requires a skilled nurse with experience in hospice care and community health. The Hospice Triage LVN collaborates with the interdisciplinary team to ensure seamless patient care, responding to triage needs and facilitating appropriate care coordination. This professional nurse uses community resources to develop tailored care plans for patients and families, meeting their needs both within the office and through home visits as required.
