Salary: $30 - $40 / per hour
Maxim Healthcare Services is seeking a Nurse (LVN) in the North Bay area to join our Home Health New Graduate Nursing Program. The Novice Nurse (< 12 months of experience) is responsible for providing and documenting skilled nursing care in accordance with care developed for each individual patient in accordance with physician’s orders when applicable.
Within this program, you will be gaining hands-on experience by working 1-on-1 with a patient within the home setting. You will have the opportunity to learn direct patient care, care management, vent/trach/g-tube management, and critical thinking with the 24/7 support of the clinical supervising team. Our goal is to best prepare our Novice Nurses for the nursing field, whether that be in Home Health, Pediatrics, ICU, Case Management, etc.
Essential Duties and Responsibilities:
Re-evaluates patient nursing needs regularlyUtilizes the nursing process to plan, implement, and evaluate patient/client care and/or contracted servicesInitiates the plan of care when directed and requests from prescriber necessary revisions as patient status and needs changeIncorporates therapeutic, preventive, and rehabilitative nursing actions based on nursing diagnosis and established goalsAssures patient safety at all timesIntervenes as applicable to changes in condition and reports such changes to appropriate supervisor and/or physicianAdministers medications and treatments as prescribed, as well as, monitors and documents effectiveness of interventionMonitors the safety and effectiveness of the environment and equipment including universal precautionsReviews plan of care, program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs as applicableDocuments all physician collaboration/coordination of care in the medical record and communicates this information as neededSupports professional practice by observing the following;Nurse Practice ActStandards of Nursing PracticeStandards of Patient CareStandards of Accrediting Body (ACHC, Joint Commission, etc.)State and Local LawsPatient Bill of RightsMaxim Healthcare, Nursing, and Client’s PoliciesAdheres to Maxim policy and procedure and applicable protocolsAttends case conferences, team meetings, education in-services as requiredPerforms other duties as assignedMinimum Requirements:
Graduate of an accredited school of nursingLicense RequiredA minimum GPA requirement is expected.
