All while understanding that **together** we are even better. **Schedule:** Full Time **Shift** : Monday-Friday 8am-5pm shift may vary based on business needs **Location:** 151 VICTORIA COMMONS BLVD, Deland, 32724 **The role you’ll contribute:** Provides direct and indirect services to specific age groups in the outpatient rehabilitation settings. Utilizes process improvement in achieving the goals of the Sports Med & Rehab Department. Accepts the responsibility to practice autonomously and collaboratively in all practice environments to provide best practice to the patient/client. Autonomous practice is characterized by independent, self-determined, professional judgment and action. Adheres to the Hospital Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies, and accrediting bodies. May supervise Occupational Therapy Assistants, volunteers, technical aides and interns. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. **The value you’ll bring to the team:** + Determines Occupational Therapy Diagnosis and establishes plan of care, make recommendations for referral if treatment needed outside of scope of Physical Therapist practice. + Engages in promoting evidence-based standards of care and lifelong learning through autonomous and collaborative participation in research, journal club/reviews, teaching/mentoring/CI and/or clinical practice education within a peer group. + Performs evaluations and re-evaluations according to standards and recommended techniques. Establishes, implements, and adapts an individualized treatment plan of care (including appropriate goal setting, treatment activities and frequency). Discharges patients when maximum benefit has been achieved and/oraccording to standards of practice and payer guidelines. + Participates in practice changes as a result of performance improvement activities. Supports quality standards and initiatives set by the department. + Regularly participates in marketing to physicians/physician practices to build referral relationships with a minimum of two contacts per month.Supports consumer marketing and public awareness through participation in community events, educational lectures/speaking and/or other extension of clinical expertise to the community as approved by manager or director. **The expertise and experiences you’ll need to succeed** **:** **Minimum qualifications:** + Bachelor’s degree in Occupational Therapy and five or more years of experience as an occupational therapist, OR + Master’s in Occupational Therapy and one or more years of experience as an occupational therapist + Current valid State of Florida license as an Occupational Therapist + Basic Life Support (BLS) Certification **Preferred qualifications:** + Previous experience or clinical rotations in an outpatient setting This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
