JOB QUALIFICATIONS

·       5 years’ experience in health care, preferably home health care & hospital.

·       Current license in good standing as a Regulated Health Professional, in the province of Ontario.

·       Graduate of an accredited and approved program.

·       Completed Continuing Education Requirements (CEUs) as required by regulation

Solid knowledge of the principles, practices, and methods of community Rehab  and service delivery. Knowledge of Ontario community health system navigation and patient discharge care planning recommended. Exceptional business acumen and ability to collaborate with hospital partners, community partners and stakeholders at all levels. Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format

·       Exemplifies characteristics of Bayshore Health Care\: compassion, excellence and reliability.

RAI Tool experience and knowledge an asset. Evidence of current CPR certification Have valid driver’s license and reliable vehicle for commuting

·       Knowledge of health & safety and infection prevention and control.

·       Demonstrated leadership skills

·       Working knowledge of business office technologies\: PC/laptop with Microsoft Office applications, smart phone, website/internet, email.

·       Ability to read, write and effectively communicate in English
