Role Description:

At Oak Street, Medical Management is focused on supporting high-quality, cost-effective care outside the four walls of primary care clinics. This involves a deep understanding of value-based care, specifically full-risk / capitated arrangements, along with the managed care levers available to drive superior clinical and financial results.

The Manager, Network Management will be instrumental in building and managing collaborative partnerships with high-volume acute hospitals, skilled nursing facilities, inpatient rehabilitation facilities, long-term acute care hospitals, home health agencies, and inpatient staffing agencies. They will be an effective cross-team leader, working side by side with Treehouse/corporate and field leaders and staff.

Content areas for the Manager will include, but not be limited to, value-based care, acute and post-acute care, contracting, relationship management, transitional care, utilization management, and care management. This will require building relationships with internal and external executive leaders, outlining collaborative structures with clear roles and responsibilities, managing performance, and ensuring our collaborative partnerships generate shared clinical and financial value for all stakeholders, especially our patients.

This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently within Oak Street Health and CVS Healthspire’s Risk Bearing Entity.

This role reports up through the Healthcare Delivery organization at CVS Health and will work closely with leaders across Oak Street Health, the Risk Bearing Entity, and the broader CVS enterprise.

Core Responsibilities:

Serve as primary lead / point of contact for developing collaborative relationships with high-volume acute hospitals, skilled nursing facilities, inpatient rehabilitation facilities, long-term acute care hospitals, and home health agencies

Serve as external champion of Oak Street Health and CVS Healthspire, able to clearly describe the value proposition of the Healthcare Delivery organization 

Manage end-to-end relationship management process including:

Identifying and prioritizing prospects

Crafting a clear pitch based on shared needs / value

Driving alignment and formalization of division of responsibilities, including partnering with Legal team on agreements (LOIs, MOUs, contracts) as needed

Supporting implementation and go-live readiness

Managing performance and iterative feedback

Develop and maintain structured internal tracking system to organize numerous active partnership processes at once and facilitate regular reporting on statuses/progress

Partner with key internal stakeholders on transitional care and utilization management teams to drive successful care coordination activities

Work side by side with Oak Street, RBE, CVS leaders to ensure effective implementation of clinical and business priorities

Accountability to key medical management metrics, including quality measures, utilization measures, and financial measures

What are we looking for?

5-7 years of applicable experience in healthcare / population health / working with healthcare providers in a relationship management function (e.g., business development); or other similar fields

Inpatient care delivery experience (clinical, such as case management, or administrative) a bonus

Existing relationships with major health systems across U.S. a bonus

Ability to build the trust and confidence of a wide range of stakeholders, including C-Suite health system / provider executives

Strong leadership skills and strong influencing skills

Incredible level of organization and detail orientation

Analytical and data savvy. Ability to develop business cases, ROI analysis, and financial analysis

Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives

Expert in MS Office, Google Apps, project management software

Pleasantly persistent approach

Humility

Quick learner comfortable with a high level of ambiguity and independence

Graduate degree preferred but not required (MBA, MPH)

Bachelor’s degree required
