Role Description
The Manager of Population Health will immediately become an important contributor to the Oak Street Health team. S/he will execute a set of responsibilities to enable the success of our clinical model, which is core to Oak Street's success.
Responsibilities
Specific responsibilities include:
Key focus on health outcomes and controlling medical costs in our patient populationOwning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics)Leading the clinical development of new care programs, including coordinating and synthesizing feedback from care teams and reviewing evidence-based guidelinesReporting and analytics in support of our Care Teams (physicians, nurses, care managers)Establishing and maintaining relationships with both internal and external stakeholdersOther duties, as assignedThe role reports to the Director of Population Health Strategy & Analytics and has significant
opportunity for rapid advancement.
