Position Summary:
The Manager of Social Services, under the direction and supervision of the RCF Administrator, is responsible for provision of social work, therapeutic recreation, and admissions functions for the Residential Care Facility. This role includes psychosocial assessment, developing and implementing individualized social service care plans for patients and families served by the Agency to maximize patient potential for health and functioning. Coordinates social service activities within the department. Provides intervention and maintains competence in these areas. The Manager of Social Services serves as a member of the interdisciplinary care team contributing to the understanding of the social, emotional, and economic factors that pertain to the resident’s care. The Director of Social Services supports the philosophy and objectives of the hospital, and all State and Federal guidelines.
Education, License & Cert:
Masters, Social Work
The employee is expected to obtain a New York State required social work license within six months from the date of hire.
Experience:
The Social Worker must understand and demonstrate the social work roles of advocate, mediator, organizer, enabler, teacher, and facilitator as they are central to the provision of effective and comprehensive social work service. Ability to do oral presentations and teaching for groups and individuals. Demonstrates commitment to quality patient care. Current CPR certification is required.
Essential Functions:
1. Compliance with rules and regulations of all federal, state, and local governments is a requirement.
2. In response to a request from the doctor, participates in the development of the plan of treatment.
3. Performs psycho‐social assessments. Based on assessment visit, will develop a plan of social services. Works with the patient and family in implementation of the plan.
4. Assists the physician and other team members in understanding the significant social and emotional factors related to health problems of an individual patient or family.
5. Prepares clinical progress notes on each patient.
6. Participates in team and interdisciplinary conferences.
7. Responsible for setting up case conferences for social work cases.
8. Provides information to medical and nursing staff relating to home situations to enable better understanding of the illness, including background information or social history.
9. Plans constructively with patient and family to restore patient to optimum social and health adjustment within his/her capacity.
10. Makes appropriate referrals.
11. Assists with problems which involve financial, social, or environmental difficulties and refers to appropriate community resources.
12. Responsible for department quality improvement.
13. Evaluates productivity of social services, therapeutic recreation, and admission.
14. Writes policies as needed.
15. Performs competency and performance evaluations on social service, therapeutic recreation, and admission staff.
16. Orients new social service, therapeutic recreation, and admission staff.
17. Supervises social service, therapeutic recreation, and admission staff.
18. Plans departmental budget in conjunction with administration.
19. Cooperates with community agencies to coordinate activities involving patients' health and welfare needs and participates in conferences with other agencies to discuss more effective ways of working with multi‐problem families.
20. Willingness to abide by the policies of the Guthrie Cortland Medical Center.
21. Understands medical, psychiatric terminology, and principles, to communicate with people having diverse backgrounds and education levels; prepare reports to evaluate and present workable service plans.
22. Keeps informed of fire, safety, and disaster programs.
23. Oversees the planning and organizing of a variety or recreational, social, cultural, and other purposeful activities for the residents.
24. Oversees documentation completion for all social services, therapeutic recreation, and admissions staff.
25. Conducts and oversees the resident council.
26. Oversees admissions process to ensure all documentation is completed upon admissions such as admissions agreements.
27. Work with the Business Office and Insurance Specialists to ensure documentation is collected for Medicaid Applications.
23. Attends designated inservice programs.
Other Duties:
1. Other duties as assigned.
The pay for this position ranges from $35.15-$55.67 per hour.
