Manager-Ambulatory/Women's and Children's Care Management - Population Health - Day
Location: WellSpan Health, York, PA
Schedule: Full Time
Hours: M-F Day
Full Time: 40 hours/week
General Summary
Works in a team leadership model to develop and maintain operations that are consistent with Hospital, Medical Group, Population Health, System and community needs. Provides direction to staff regarding care management population management, and social services. Provides input into the budget management of resources and works collaboratively with leadership to oversee the operations of ambulatory care management. Ensures compliance with NCQA standards.
Duties and Responsibilities
+ Oversees the hiring of ambulatory care management staff. Trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions.
+ Participates in developing budgets.
+ Collects and tracks data and patient outcomes to identify needs in program planning and facilitates program development. Supervises patient care, family communication, and record keeping.
+ Collaborates with other care management programs and community agencies with which patients and families are involved.
+ Collaborates with leadership to support operational goals and the Population Health Strategy and support of those goals and participates/ represents case management with community committees.
+ Coordinates and implements the development of recommended policies and programs. Coordinates, establishes and monitors achievement of departmental goals and objectives. Responsible for achievement of assigned NCQA standard compliance and compliance with all applicable State and Federal laws, as well as achievement of assigned incented health outcomes.
+ Actively seeks out technology advancements and process workflow improvements, ensuring team members are working to the top of their license to provide comprehensive care to the identified patient population.
+ Ensures workflows and documentation are set up to succeed in maximum billing, reimbursement, and patient outcomes.
+ Engages with staff to develop lean daily management process to access opportunities for improvement and empowers team to address. Conducts employee/ leadership rounding with staff to identify and resolve barriers for staff.
Travel Requirements:
+ Estimated Amount: - May need to travel to all counties across WellSpan's footprint.
Qualifications
Minimum Education:
+ Bachelors Degree Required
+ Masters Degree Preferred
Work Experience:
+ 3 years Supervisory Experience. Required
+ Population Health Experience. Preferred
+ Case Management Experience. Preferred
+ Experience with Healthcare Management and/or Ambulatory Care preferred
+ Experience with grant management preferred
+ Experience with evidence-based practices for maternal and child health, including prenatal and postpartum care, pediatric complex care, and addressing social determinants of health required
Licenses:
+ Licensed Registered Nurse Upon Hire Preferred
