Manage the Care Management Department to support the acute care operation SW care management services. Responsible for resource management, customer relations and operational standards which include case management, utilization management and transition of care. Provides the leadership and direction to care management staff, such as RNs, MSWs, and other support staff. Develops programs and processes that support the integrated care of patients to ensure that their care can be optimally supported by the medical, social, and community resources available to them. Oversees the care management planning process to ensure safe, supportive, and timely service for patients ensuring coordination of care. Maintains knowledge of applicable regulations and rules that affect the provision of services.
Adheres to all budgetary and productivity goals/standards. Ensures the attainment of department and hospital goals, policies and procedures. The Manager promotes interdisciplinary collaboration, fosters teamwork, and champions service excellence, and facilitates excellent communication, collaboration and safe transitions in care.

Required Qualifications:
Bachelor's Degree in Nursing. Upon hire: Washington Registered Nurse License. 10 or more years Multidisciplinary clinical experience including 4 years case management experience and 2 years management level experience. Experience in utilization management, case management, discharge planning, disease management and other cost/quality management program. Background in business planning and targeted clinical outcomes. Experience in change management.Preferred Qualifications:
