The WellBe care model is a Physician Led Advanced Practice clinician-driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end-of-life care.  WellBe's physician/advanced practicing clinician-led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

GENERAL SUMMARY

The Manager, Care Model Implementation, Clinical Programs participates in cross functional internal and external teams to define, create, pilot, and launch various projects and programs that benefit the WellBe Senior Medical model.   As the Manager, Care Model Implementation for the clinical department, you will function as a “SME” leading initiatives in a fast-paced environment that leads to progressive decision making based on project risk, timelines, budget, and on time deliverables.  Initiatives include but are not limited to transitional care, hospital prevention and ED diversion, end of life and palliative care, cancer care, pharmacy, behavioral health and social services, clinician education, and value-based care model execution. 

 

This position requires self-direction, time management, critical and analytical thinking, strong written and verbal communication, agility, organizational excellence, reporting, high degree of healthcare knowledge, and finally project/program effectiveness.

Job Description Independently leads and motivates teams of internal and external projects in a fast-paced start-up environment Function as a subject matter expert (SME) leading effective coordination between organization/department process flows, requirements definition, planning, design, rollout and monitoring. Meet with stakeholders to define project scope, business and operational requirements, and produce timelines that align to client and business needsAbility to anticipate issues and work toward a solution by building consensus within the project team.Participates in projects across the organization and be ultimately responsible for the delivery of high-quality deliverables that power client satisfaction, meet business requirements, and balance client economicsInterface heavily with marketing, technical, clinical, patient experience and market leadership to assure that the solution is understood and addresses the customer's business requirementsManage the execution of work, which includes tracking team progress, routing deliverables for approval, monitoring scope, roll out and training. Actively monitor project risks/issues, escalate in a timely manner, and drive teams to effective solutionsActively monitor project risks/issues, escalate in a timely manner, and drive teams to effective solutionsDevelop and manage detailed project plan(s) that integrates key activities, timelines, resources, budget, and track the critical path for assigned project(s).Ensure uniform processes/documentation between projects including project meeting organization, issuing agendas, documenting project team meeting minutes, tracking, reporting progress on projects and proactively managing follow up and resolution on action items.Focus on clinical strategies (transitional care, ED diversion, end of life, pharmacy, behavioral health) that positively impact population health driven outcomes through improving clinical health outcomes and care coordinationIn collaboration with the department heads and team members develop, implement, and monitor an integrated operational plan that supports the successful execution of project/program strategy to meet goals for assigned project(s) in accordance with the project goalsDevelop and implement change management strategies and plans that maximize employee adoption and usage of required changesReviews, assesses and interprets data from ongoing quality measures.Develops and implement strategies related to clinical quality improvement Own strategic efforts to improve the clinical operations and clinical protocols from a growth, performance, efficiency, and member value perspective - this includes defining the problem and analyses, and synthesizing learnings in a way that drives organizational action & changeLead market research and competitive analysis for valuable clinical care models Facilitate decision-making process within the team.Foster open dialogue within the team and build positive team spirit to overcome obstacles.Facilitate resolution of conflicts between functional groups and external collaborators while maintaining objectivity.Support contractual and intellectual property requirementsOther tasks needed to accomplish team’s objectives/goals

 

Job Requirements

Educational Requirements:

BS/BA in relevant program, or equivalent work experienceMPH or MHA preferredPMP certification desirable5+ years of experience delivering operational projects/programs with heavy technological and product development needs3+ years’ experience in healthcareClinical experience (RN, SW, PT/OT, etc) preferredSolid understanding of project management methodology, including the ability to identify and resolve issues, manage risk, and develop detailed work plans and specifications
