We offer the following distinctive services at Cedar Hills Hospital: Mental Health Services Substance Abuse Treatment Program Freedom Care Military Program The Women’s Program Co-Occurring Disorders Program Cedar Hills Outpatient Services Cedar Hills Hospital | Portland, OR | CedarHillsHospital.com THE MANAGER OF SOCIAL SERVICES The Manager performs social services functions to include assisting patients in meeting their biopsychosocial needs while enabling them to access through therapeutic treatment skills to achieve their optimal level of emotional health, including providing basic consultation, teaching, complete specialized clinical social work services for patients/families receiving care, crisis intervention, short-term and long-term family therapy, and providing information and referrals as needed. The Manager is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. The Manager is a solid leader, shows initiative and creativity, shows a high level of organizational & multi-tasking skills, detail-oriented, patient, shows high level of stress-management ability, performs at a high level of accuracy, displays excellent clinical knowledge and skills, and is a positive role model. A Wednesday to Sunday schedule is preferred with flexible hours available. Experience and Licensure: 4 years minimum with LCSW strongly preferred.
