The Manager is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. The Manager is a solid leader, shows initiative and creativity, shows a high level of organizational & multi-tasking skills, detail-oriented, patient, shows high level of stress-management ability, performs at a high level of accuracy, displays excellent clinical knowledge and skills, and is a positive role model. A Wednesday to Sunday schedule is preferred with flexible hours available.
