JOB SUMMARY
This job manages a team of Population Health Pharmacists and support the team in the implementation and execution of the Population Health Pharmacy Strategy. Manage team efforts to improve provider performance under value based reimbursement contracts, STARs performance, HEDIS performance, Risk Adjustment documentation and any other risk based contracts developed by Highmark, Inc. The incumbent will report to the Director of Value-Based and Population Health Pharmacy.
ESSENTIAL RESPONSIBILITIES
Perform management responsibilities to include, but are not limited to: involved in hiring and termination decisions, coaching and development, rewards and recognition, performance management and staff productivity. Plan, organize, staff, direct and control the day-to-day operations of the department; develop and implement policies and programs as necessary; may have budgetary responsibility and authority.Develop and Execute employee engagement plan for team including assessment of team skills and performance. Determine continuous improvement and training programs for team. Maintain team performance dashboards and development performance improvement efforts when necessary.Implement the team's strategy under the direction of department Director for improving provider scores on metrics related to pharmacy under the VBR contracts. Determines team deployment plan and maintains team provider assignments. Serve as business lead for assigned department initiatives including but not limited to department C2V efforts, development and execution of population health pharmacy playbook, and management of pharmacy students.Work in a matrixed fashion with Highmark, Inc's pharmacy department to support overall health plan pharmacy and VBR efforts. Includes coordination, collaboration, and communication with other enterprise pharmacy departments on correlating initiatives. Other duties as assigned.EDUCATION

Pay Range Minimum:
$92,300.00Pay Range Maximum:
$170,800.00Base pay is determined by a variety of factors including a candidate’s qualifications, experience, and expected contributions, as well as internal peer equity, market, and business considerations. The displayed salary range does not reflect any geographic differential Highmark may apply for certain locations based upon comparative markets.
