As a Manager of Care Navigation, you will:
• Support departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate
• Lead team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner
• Lead team to collaborate with the acute care teams, post-acute care teams, and physician practices to ensure patients are navigated to the most appropriate level of care in outpatient setting
• Ensure active participation in Interdisciplinary Rounds and other patient care conferences
• Lead delivery of care provided by Care Navigation team to ensure that patients’ needs are addressed, and that documentation meets standards
• Implement the departmental strategic plan
• Collaborate with Utilization Review leadership to understand trends of discharge barriers and denials
• Facilitate the development of systems, processes and PI that drives optimal performance for the department
• Oversee and evaluate the performance of the Care Navigation team
Qualifications include:
• Bachelor of Science in nursing required, Master’s degree preferred
• 3 – 5 years’ experience in patient navigation, discharge planning, or care coordination
• At least one year of prior leadership experience required
• Maryland RN licensure OR Active Maryland license as LCSW-C or LCSW with plans to achieve LCSW-C in 1 year
• Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance as directed by AHC .
• Active America Heart Association Basic Life Support (BLS) Certification
Work Schedule:
Monday through Friday with a minimum of 8 hours per day; on-call/oversight on weekends and holidays

Pay Range:
$89,356.80 - $134,035.20In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.
