In collaboration with the AVP of Utilization and Care Management leader, this professional manages a team of staff, composed of social workers and assistant staff. This team is responsible for providing care management services to the Behavioral
Health patient population that consists of care coordination, care progression, discharge planning, and social services. This team also works in collaboration with the medical and nursing staff to manage length of stay and the care of patients at risk for readmission. With the AVP, the Manager leads the team in continuous process improvement activities to ensure that patients receive cost-effective, high quality, and safe care. This position provides leadership and clinically supervision for all Licensed Social Workers, case management assistants, and social work students within the inpatient psychiatric settings of Sinai Hospital. Responsible for leadership of the teams' provision of group and individual clinical care and programming, coordinating family meetings, biopsychosocial assessments and care leading up to and including discharge planning. Ensures services are
interdisciplinary treatment plan-based, and consistent with measurable patient goals, as well as departmental and organizational goals, mission, and vision. Acts as a resource to patient/client, families, hospital staff, and other LifeBridge Health entities on the clinical care and community resources for this patient population.   REQUIREMENTS: Master of Social Work degree Current Maryland LCSW, LCPC or Psychology license 5–7 years Inpatient Social Work
