Overview Manager of Social Work Services On-site (Farmingdale) Good Shepherd Hospice, a member of Catholic Health, promotes the physical, emotional and spiritual well-being both of the person completing life's journey and their family members, providing comprehensive and compassionate Palliative and end-of-life care of the highest quality. Good Shepherd helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most. Our team of professionals, caregivers and trained volunteers are dedicated to providing both medical and emotional support to patients and families. We currently employ more than 250 staff members and have a volunteer core of over 100 dedicated people. Job Details The Manager of Social Work Services is responsible for the oversight and supervision for Social Work and Psychosocial Support Services for Good Shepherd Hospice patients and assures the delivery of care in accordance with Hospice Regulations. This managerial role will also provide mentorship and serve as a resource for clinical staff. Primary responsibilities include, but are not limited to: Coordination of all Social Work activities in Community and Inpatient Hospice. Provide clinical supervision and leadership to the Social Work staff through ongoing performance measurement and evaluations. Supervise Electronic Medical Records (EMR) documentation for appropriateness, timeliness and plan of care follow up. Participate in the Interdisciplinary Team meetings and serve as a resource for psychosocial support services to patients and families. Identify and develop enhancements to the social work program, improving quality of care, patient outcomes program deliverance and work flow. Assures compliance with all applicable Department of Health and Joint Commission regulatory requirements Develops strategic initiatives/goals in conjunction with other Good Shepherd Hospice Clinical Management teams. Minimum Requirements: Master’s degree in Social Work from an accredited college/university Current and valid NY State license to practice as a Social Worker. LCSW preferred. Minimum of 2 years’ experience in healthcare, specific to counseling services for severely and/or terminally ill patients Thorough knowledge of Medicare Conditions of Participation, Hospice concepts, philosophy, policies and procedures Demonstrated ability to identify the needs of patient populations and plan for delivery of competent-quality care delivering successful outcomes. Strong leadership, oral, written and presentation skill Salary Range USD $100,000.00 - USD $115,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
