OverviewThe Master of Social Work will work to identify and intervene with at-risk populations to mitigate psychosocial barriers to optimal care outcomes. Social workers provide services to children, adolescents, and adult patients/families/staff at Children's Mercy and complete comprehensive psychosocial assessments and develop treatment plans in conjunction with internal and external team members.
Shift: Wednesday, Thursday (4p - 12:30a), and Saturday and Sunday (5:30pm - 2:00a). This role will work at the main campus in KCMO.

ResponsibilitiesPsychosocial Assessment and Treatment Planning:
How work is completed:
• Assess patient and family psychosocial risk factors through evaluation of prior functioning levels, availability of support systems and coping skills utilizing knowledge of the biopsychosocial model, human development, cultural humility, trauma informed care, strengths perspective, and social work ethics with the focus on identifying barriers and risk factors to create treatment plans that maximize health outcomes. Reassessments are conducted as needed or requested. All assessments are guided by hospital policy, regulatory requirements and applicable laws.
• Identify, assess, and report suspected abuse and neglect in collaboration with the multidisciplinary team, including patient, when applicable, and family as required by law and hospital policy with particular focus on risk reduction and prevention. Reassessments are conducted as needed or requested.
• Perform targeted mental health assessments in response to positive mental health screens, patient need, and staff request. Targeted mental health assessments may include:
Timely response Targeted assessment guided by the literature and best practice standards Safety assessment, means restriction education, and safety planning Care coordination to ensure seamless care transitions Collaboration with patient, family, and multi-disciplinary team Documentation in accordance with department policy Follow up services and support per department protocol to ensure patient is connected with appropriate services• Utilize psychometric scales to objectively assess patients and families and guide treatment planning.
• Provide consultation, and professional support to team members and colleagues, including internal and community partners, to ensure a collaborative approach to population health.
• Inpatient Specific: Screen admissions to risk stratify and prioritize patient needs. Patients with the following needs are the top priority for assessment:
Psychosocial barriers to hospital discharge Readmission within 7 or 30 days Inappropriate use of the Emergency Room Lack of connection to primary care or medical homeClinical Services and Intervention:
How work is completed:
• Develop an effective working relationship with patients and families to engage, collaborate, educate, advocate and enhance coping skills in order to improve functioning with respect to self-determination.
• Create and implement timely, comprehensive, evidenced informed, and outcome focused interventions for patients and families.
• Provide psychoeducation and support to patients and their family regarding adjustment to illness, both chronic and acute, changes in living situations, disability, hospitalization, death and dying, child/adult abuse and neglect, intimate partner violence, social, emotional and mental health needs and access to resources.
• Perform crisis intervention, de-escalation and safety planning to ensure the safety of patients, families, and staff.
• Utilize social work theory and best practices, which may include but are not limited to:
Family Systems Theory Person in Environment Motivational Interviewing Solution-Focused Brief Therapy• Collaborate with patient, family, and medical team throughout the transition planning process.
Care Coordination:
How work is completed:
• Conduct pre-visit planning in order to identify and mitigate psychosocial barriers to care.
• Utilize a family centered care approach to engage patients and families in the care coordination process.
• Demonstrate robust knowledge of internal and community resources to benefit patients and families through the coordination of services.
• Conduct timely referrals to appropriate community services based on identified needs to ensures continuity of care.
• Provide appropriate care coordination to optimize patient well-being across the continuum of care.
• Proactively engage and collaborate with internal and external stakeholders, along with the patient and family, to ensure all members of the team are aware of the care plan and facilitates ongoing communication among the team to monitor progress toward goals and identify and mitigate barriers to successful transitions.
• Attend rounds, care plan meetings, care conference and disciplines specific meetings on complex cases to ensure seamless transitions of care, and include internal and community team members when appropriate.
• Provide education to patients and families on disease management and optimal utilization of health care services.
• Identify and report care transition barriers and promotes resolution within the hospital system and/or community.
• Provide timely care transitions, or handoff, to internal and community partners to ensure seamless care transitions for patients and families.
• Collaborate with Patient Access regarding the patients financial status when appropriate to ensure maximum coverage for the patient and the hospital.
• Recognize legal responsibilities and report areas of concern to manager.
Practice Management:

• Maintain timely and accurate documentation of all interactions in the electronic medical record in accordance with department policy.
• Record clinical productivity data consistently and accurately, and in accordance with department standards.
• Accurately record data and contributes to department and hospital data reporting as requested.
• Demonstrate responsibility, efficiency, and good organizational skills.
• Assist co-workers or others in the department as needed or requested.
• Provide on-call coverage that responds to the critical needs of patients, families and department.
• Identify opportunities for and provides professional consultation to staff and community professionals regarding psychosocial issues impacting children’s health.
• Actively engage in the Children’s Mercy Lean System including participation in huddles, problem-solving methodology (STP, A3), Kanban System and submission of data as requested.
• Actively participate in psychosocial critical incident response plan.
• Demonstrate expertise and leadership to colleagues and team members about the social determinants of health, psychosocial care and the social work profession.
• Attend and contribute to the success of department and team meetings.
• Serve on hospital and department committees related to practice area as needed or requested.
• Participate in professional development, research, and quality improvement opportunities as determined by supervisor and staff.
• Complete all required hospital, department, and disease specific education within established time frames.

Our pay ranges are market competitive. The pay range for this job begins at $28.25/hr, but your salaried offer will be determined based upon your education and experience.
