POSITION SUMMARY: Person in this position will play a critical role in delivering comprehensive healthcare services to individuals seeking assistance with substance abuse disorders. Working within a multidisciplinary team, the APRN assesses, diagnoses, and treats patients, employing a holistic approach that integrates medical, psychological, and social aspects of care. The APRN is responsible for providing medication-assisted treatment (MAT) and coordinating various therapeutic interventions to support patients on their journey to recovery. Essential Job Functions include those listed below. ESSENTIAL DUTIES AND RESPONSIBILITIES: Conduct thorough assessments of patients to determine eligibility for medially assisted treatment. Gather and analyze medical histories, substance use histories, and relevant psychosocial information. Collaborate with other healthcare professionals to ensure a comprehensive understanding of each patient’s needs. Prescribe and manage medications for substance use disorder treatment, such as opioid agonist therapy (e.g., vivitrol, buprenorphine). Monitor medication adherence, efficacy, and potential side effects. Adjust medication regimens based on patient response and clinical judgment. Collaborate with treatment team to develop and implement individualized treatment plans. Participate in case conferences and treatment team meeting to ensure coordinated and holistic care. Communicate effectively with patients and their families to foster engagement and adherence to treatment plans. Provide education to patients on the nature of substance use disorders, treatment options, and recovery strategies. Conduct counseling sessions, addressing both medical and psychosocial aspects of addiction. Foster a therapeutic alliance with patients to support their commitment to recovery. Maintain accurate and up-to-date patient records, documenting assessments, treatment plans, and interventions. Ensure compliance with legal and regulatory requirements related to medical record documentation. Participate in quality improvement initiatives to enhance the effectiveness of the Medically Assisted Program. Stay abreast of evidence-based practices and contribute to the integration of best practices into the program.
