Maternal Child Nurse Navigator

Summary

Serves as a consistent care coordinator throughout the continuum of care assessing the physical, psychological and social needs of the patient. Monitors the patient and triages needed services across all service lines and community resources while ensuring proper access to these services. The results are enhanced patient outcomes, increased satisfaction, and reduced costs of care. Participates in internal and community-wide quality improvement initiatives.

 

Education

Must have a BSN from accredited program. Acceptable accrediting agencies are Commission on Collegiate Education in Nursing (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA).

 

Licensure/Certification

Valid Indiana Registered Nurse license required. Certification in related field preferred. Must have CPR certification.

 

Experience

Must have three years’ recent experience in maternal child health nursing.  Experience in obstetrics, pediatrics, psychiatric nursing, or case management preferred.

 

Other Qualifications

Strong verbal and written communication skills are required. Must be motivated, flexible, and work well independently and as a part of a team. Demonstrates the ability to build and maintain good working relationships across the health system, both internal and external. Exhibits sound judgment, adapts to change, and works efficiently. Displays initiative by looking for new opportunities and working towards continuous quality improvement. Demonstrates cultural competence and enthusiasm in working with underserved populations.
