MCD Care Manager RN
**Responsibilities** Coordination of a patient's clinical care needs from either an inpatient hospitalization to outpatient; from a post-discharge facility to a home or assisted living facility; and/or coordination of resources to assist patients back into a clinical setting with PCP. Includes, but is not limited to, insurance authorizations (medication, transportation, alternate level of care), coordination of care to alternate levels of care (skilled nursing homes, Inpatient rehab, home, including transportation), initiating and organizing hemodialysis, coordinating inpatient hospice, home hospice or skilled nursing with hospice; Palliative care and obtaining information and connecting patients to appropriate outpatient regional resources **Job Title: MCD Care Manager RN** **Location: St. Lukes Medical Towers** **Department:** **Health Care Service Lines** **Job Type: Full Time** **Salary Range: $81,646 - $100,857 (Pay is based on several factors including but not limited to education, work experience, certifications, etc.)** **Qualifications** **Education** Required: Associate degree in Nursing Preferred: RN Licensure with bachelor’s in social work **Experience** Required: Minimum 5 years of Discharge Planning Experience. Preferred: Coordinated Patient Discharge for 2 or more years. **License(s)** Required: Valid and current Texas RN license. Current BLS issued by the American Heart Association. Preferred: RN and LCSW **Special Skills** Required: Care coordination with Hospital and Outside the Hospital.
