Martine Center is hiring an MDS Assessor to work in our Skilled Nursing Facility. The ideal candidate will be a Registered Nurse (RN) with prior MDS 3.0 experience, and Long Term Care experience.

DUTIES:

Completing accurate assessments, MDS & care plans as assignedInitiating care plans and supporting activities as assignedCreating and distributing monthly care plan calendars in a timely fashionMaintaining & updating all care plans and assessments as requiredMonitoring & auditing clinical records, ensuring accuracy & timelinessInforming DON of persistent issues related to non-compliant documentationProtecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

MUST HAVE PRIOR EXPERIENCE WITH MDS 3.0Valid NYS Registered Nurse (RN) LicenseLong Term Care Experience RequiredMust be highly organized, professional & motivatedShould have solid computer skillsExcellent communication skillsShould be friendly and a team worker
