Ontario Center is hiring an MDS Coordinator to work in Canandaigua, NY.

DUTIES:

Completing accurate assessments, MDS & care plans as assignedInitiating care plans and supporting activities as assignedCreating and distributing monthly care plan calendars in a timely fashionMaintaining & updating all care plans and assessments as requiredMonitoring & auditing clinical records, ensuring accuracy & timelinessInforming DON of persistent issues related to non-compliant documentationProtecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

MUST HAVE PRIOR MDS 3.0 EXPERIENCEValid New York RN LicenseLong Term Care Experience Required!Must be highly organized, professional & motivatedShould have solid computer skillsExcellent communication skillsShould be friendly and a team worker
