MDS-Nurse Job Description Location: 119-Southern Utah Veterans Home - Ivins, St. George, UT, 84738 Job Summary:

The MDS-Nurse is responsible for coordinating and completing the Minimum Data Set (MDS) assessments for all residents in the facility. The MDS-Nurse will work closely with the interdisciplinary team to ensure accurate and timely completion of the assessments. The MDS-Nurse will also be responsible for ensuring that all assessments are completed in compliance with state and federal regulations.

Key Responsibilities: Coordinate and complete MDS assessments for all residents in the facility Work closely with the interdisciplinary team to ensure accurate and timely completion of assessments Ensure that all assessments are completed in compliance with state and federal regulations Review and analyze resident medical records to ensure accuracy of information Communicate with physicians and other healthcare professionals to obtain necessary information for assessments Participate in care planning meetings to ensure that resident care plans are based on accurate and complete assessments Provide education and training to staff on MDS assessments and regulations Participate in quality improvement initiatives related to MDS assessments Qualifications: Current nursing license in the state of Utah Minimum of 2 years of experience in long-term care Experience with MDS assessments and regulations Strong attention to detail and accuracy Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills
