And above all, it means exceptional care, simply delivered — and we couldn’t do it without you. **Make a Difference** The **Medication History Technician** will **obtain and document medication histories** in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards. **Your Exceptional Skills and Qualifications** · Performs thorough interviews with patients and family members with appropriate professional language. · Conducts telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication history with appropriate telephone etiquette. · Interprets and enters “Prior to Admission” medication history into the electronic medical record in an accurate and timely manner. · Coordinates responses and resolutions to issues with appropriate internal and external parties. · Regularly and appropriately communicates with supervising pharmacists for any questions and additional work assignments. · Accurately tracks interventions and productivity using department’s process. · Maintains proficiency with departmental information system technology and documentation systems. · Maintains contemporary professional knowledge base. · May represents department on various hospital committees. · Participates in evaluation of quality service, development, and implementation of improvement. · Communicates with other hospital employees and departments as necessary. · High School Diploma or GED (Required) · One of the following is required Certifications/Licensures: (Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA), Licensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA).
