Job Description

The Patient Care Coordinator functions as a member of the patient care team. This position performs clinical tasks and procedures to support the delivery of care and promote efficient use of the physician's time. Additionally, it handles all clerical duties required to complete patient Check-in/Check-out. The Patient Care Coordinator assists patients in obtaining services in a timely manner, ensures efficient utilization of the physician's appointment schedule, and coordinates patient care. This role also administers and coordinates referrals and the service authorization process, performs rooming duties, and other clinic front desk activities.

ResponsibilitiesSupport the delivery of patient care by performing clinical tasks and procedures.Complete patient Check-in/Check-out and handle all related clerical duties.Assist patients in obtaining services promptly.Ensure efficient utilization of the physician's appointment schedule.Coordinate patient care and administer referrals and service authorization processes.Perform rooming duties and other clinic front desk activities.Room approximately 25 patients daily, taking vitals and managing patient flow.Train on care coordination, including scheduling appointments with multiple departments and handling prior authorization with medication.Essential SkillsCertified Medical Assistant (CMA)Medical Assistant certification1 year of experience as a Medical AssistantAbility to take initiative and be motivatedTeam playerExperience with rooming patients and taking vitalsExperience with EPIC software (preferred)Additional Skills & QualificationsExperience in care coordination is beneficial but not requiredAbility to manage multiple tasks in a fast-paced environmentWork Environment

Employees will be working in a fast-paced Neuroscience center. The work schedule is Monday to Friday, 8:30 AM to 5:30 PM, with no weekend shifts. Temporary employees start off as 'roomers' and will be trained on care coordination tasks. The clinic is located within a renowned Neuroscience center.

Pay and Benefits

The pay range for this position is $27.00 - $30.00/hr.
