The Panel Care Coordinator functions as an intermediary between the Medical Team and the patient. In this capacity, the role is designed to provide first line support to electronic and telephonic queries. This requires the Panel Care Coordinator to proactively manage patient care for those non-office visits. As the primary conduit for the transmission of information between patient and Medical Staff, the Panel Care Coordinator will need to use their discretion to convey pertinent information to the Medical Team to correctly prescribe a treatment plan. The goal of this position is to gather, assimilate and convey the correct information to the Medical Team to provide a timely treatment plan.

Education

Required – Completion of an accredited Medical Assistant Program

Work Experience

Required – 1 year experience as a Medical Assistant

Certifications

Required – Medical Assistant Certification

Knowledge Skills and Abilities (KSAs)

Must have computer skills and dexterity required for data entry and retrieval of information Ability to understand patient panels and core medical terminology Effective verbal and written communication skills and the ability to present information clearly and professionally Familiarity with electronic medical record (EMR) systems and handling patient records Strong interpersonal skills, including the ability to work with a diverse population

Job Duties

Serves as a communication conduit between the patient and medical staff. This encompasses answering first line medical queries issued by phone or email. Gathers pertinent patient information for medical staff, routing and/or escalating calls based on patient information. Schedules and review patient care charts to coordinate future healthcare visits. Interprets patient data to route and/or escalate to the appropriate medical staff. Ensures that patient charts and medical records are timely and accurate to help provision care. Leverages medical knowledge and established procedures to evaluate patients and complete applicable scoring. Updates and maintains all registry reports while leverage this data to draft a patient care plan. Maintains and verify panel data, ensuring its accuracy and applicable to the patient care plan. Implement treatment programs as created by Medical Staff to address Patients with chronic conditions. Monitors the patient’s adherence to care plan through reassessments Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style. Other related duties as required.

The physical essential functions of this job include (but are not limited to) the following: Frequently exerting 10 to 20 pounds of force to move objects; occasionally exerting up to 100 pounds of force.
