Job Title: Medical Assistant - Population Health
Job Description
The Population Health Medical Assistant supports primary care teams either remotely or embedded within a primary care office to improve compliance on population health initiatives. These include annual wellness visits, quality care gap closure, and in-network referrals. This position does not involve direct patient care but requires executing standing orders for patients.
Responsibilities
+ Regularly review patient charts for screening needs, quality gap closure, and care coordination.
+ Work under physician licensure to place orders for screening and quality gap closures based on standing orders.
+ Perform chart preparation prior to office visits for all ACN value-based lives to identify open care gaps.
+ Manage in-network referrals for all ACN value-based lives.
+ Conduct member outreach to schedule patient visits for various health campaigns.
+ Coordinate follow-up visits for transitions of care for ACN value-based lives.
+ Transfer patient data regarding quality care gaps to ACN population health management platforms.
+ Proactively work with payer care gap and high-risk reports for all ACN value-based lives.
+ Collaborate with primary care teams and ACN clinical performance consultants to manage ACN value-based lives effectively.
+ Work cooperatively with physician office teams and ACN support teams as appropriate.
Essential Skills
+ At least three years of experience as a Medical Office Referral Coordinator, Medical Assistant, or similar role.
+ Diploma from a Medical Assistant program.
+ Experience with quality measures, including care gap closures, pre-visit planning, HEDIS measures, and patient outreach.
+ Ability to follow instructions, analyze data, coordinate tasks, and manage workload to meet business goals.
+ Excellent verbal communication skills.
Additional Skills & Qualifications
+ Experience with electronic health records and appointment scheduling.
+ Familiarity with EMR systems.
+ Basic understanding of ALOS, readmission rates, and gaps in care.
Work Environment
The role is based out of a central hub but requires floating to different provider offices across Maricopa County. Work from the ACN office in Arcadia on Mondays, clinics in the territory from Tuesday to Thursday, and possibly from a clinic or home on Fridays, depending on clinic hours.
Job Type & Location
This is a Contract to Hire position based out of Phoenix, Arizona.
Pay and Benefits
The pay range for this position is $22.00 - $28.00/hr.
