Responsibilities:
\n \n \nConducting In-Home Assessments for LT-PCS applicants and participants.
\n \n \nProviding HCBS program choice counseling to requestors.
\n \n \nProviding an overview of the Long-Term Care Program to applicants (Rights and Responsibilities).
\n \n \nDeveloping a needs-based and Person-Centered Plan of Care.
\n \n \nCompleting client monitoring surveys as part of quality management.
\n \n \n\n \n \n
Requirements:
\n \n \nBachelors in health/ human services, Social Work, Nursing or related.
\n \n \n2 years of health related social and/or human services experience.
\n \n \nMin 1-year exp working with older adults or persons with disabilities.
\n \n \nMust live in Louisiana.
\n \n \nHave a valid driver’s license.
\n \n \n\n \n \n

For this position, actual salaries will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive. The estimated salary range for this role is $19.47 per hour.
