Position Summary:

Medical Case Management is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. provide benefits counseling by assisting eligible clients in obtaining access to other public and private programs for which they may be eligible The MCM is a professional who collaborates with physicians, health care team members and patients to coordinate care and provides various social services to meet needs of patients including: biopsychosocial assessment and reassessment, care plan, insurance navigation and linkage to other resources necessary for patient engagement in care.

What you will do:  

1)Conducts biopsychosocial assessments and 

reassessments with patients at defined intervals. 
Completes acuity scale guided by assessment.

2)Comprehensive, individualized /Service Plan 
developed collaboratively with the client within 30 
days of biopsychosocial assessment or referral 
for services. Individualized Care Plan/Service 
Plan n must include: 
• Description of the need(s) 
• Action steps/interventions to address the 
need(s) 
• Treatment modality 
• Timeframes to address the need 
• Dated signatures of the client and medical case 
manager 
• Updated at a minimum of every 6 months 

3)Provides various direct services including: 
crisis intervention, coordination of services refers 
for financial assistance, makes referrals for 
community services as necessary to assist the 
patient in transitioning along the health care 
continuum

4) Provide ongoing education to clients on 
identified needs including treatment adherence
and risk reduction

5) Participates as 2nd in running monthly support 
groups receiving direction from MSW Lead 
regarding:
• Scheduling 
• Planning
• Providing direction and topics during group
• Guiding discussion
• Participating in process meetings 
• Assists in creating and following relevant 
policy/procedure 
• Reports concerns to manager and engages in 
problem solving

6) All other duties as assigned.

What you will need:  

1. Bachelor of Social Work (BSW), RN, or other related health or human service degree from an accredited college or university. OR

2. Associate’s degree with two years of providing direct HIV services and/or experience. OR

3. Certified Community Health Worker (CCHW) with 2 years of providing direct HIV services and/or experience OR

4. 4 years of experience providing direct HIV services and CCHW obtained within 1 year of hire.
