Our PACE program serves as our patients’ health insurer as well as directly provides comprehensive primary care and social services to keep them living safely in their communities. Reporting to the Chief Clinical Officer, the Medical Director will be at the center of an IDT, providing care. PACE is an ideal job for a clinician, as it allows you to spend ample time engaging with and truly caring for patients without the mis-aligned incentives of fee-for-service.

Essential Job Duties:

\n\nAssume accountability and responsibility for the medical care of WelbeHealth participants, overseeing the medical service team to promote quality and outcome goals.\nMonitor and lead WelbeHealth clinic operations to assure integration of medical services with other participant care services for participants.\nRecruit, train and supervise PACE Physicians, Nurse Practitioners, Community-Based PACE Physicians and other contracted medical staff.\nPerform comprehensive assessments for participants and coordinate with the IDT to develop a comprehensive care plan for each participant\nBe an engaged member of the IDT, communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery\nWork collaboratively with network providers and other members of the IDT to ensure effective care coordination, particularly around hospital discharge, short-term, and long-term placement\nOversee participants’ use of medical specialists and inpatient care\nParticipate in rotating night and weekend call\nReview outcome and utilization data, including adverse events, hospitalizations, and use of specialists, in collaboration with the IDT to identify opportunities for improved care and operations\n\n

Job Requirements:

\n\nSuccessful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education\nDoctorate degree\nBoard Certification – Internal Medicine or Family Medicine or or Emergency Medicine Geriatrics (Preferred)\nActive New York Physician License without restrictions\nState CDS Certification and DEA Registration\nReliable means of transportation\nLeadership experience\nExcellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families, and the IDT\nCurrent knowledge of chronic care/geriatric issues and best practices\n\n

Salary/Wage base range for this role is $300,000.00 - $330,000.00 annually + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
