Medical Director - Home Based Care

We are seeking an experienced, innovative, and compassionate Medical Director to lead and grow our care initiatives that use the virtual platform, including Mobile Integrated Health (MIH), Home-bound care, MyCare On Demand (MCOD), MyCare Advice Line (MCAL), Nurse refill line (RxRN), Advanced Home Health, and our Complex Care Clinic. As technology allows and quality care is available with the virtual platform this could expand to include future services. This leadership role is responsible for overseeing the strategic development, clinical operations, and quality management of our virtual care services and care models listed above. The Medical Director will work collaboratively with interdisciplinary teams to ensure high-quality, patient-centered care is delivered across multiple settings, including our MIH, Home-based model Complex Care Clinic, MCOD, MCAL, RxRN, Advanced Home Health, Hospital at Home, and Remote Patient Monitoring.  

The ideal candidate will be a forward-thinking physician with experience in telemedicine, home-based care models, and complex patient populations. This role is pivotal to fulfilling our mission to provide care to patients where they are, when they need it, and in the right setting. 

Key Responsibilities:

1. Clinical Leadership and Oversight

Provide clinical leadership for the virtual care platform and all the services listed to ensure evidence-based practices and high-quality care. Develop and implement clinical protocols, policies, and best practices for the virtual platform, remote monitoring, and home-based care services.  Develop and implement clinical protocols, policies, and best practices for the Complex Care Clinic.  Lead quality improvement initiative to enhance patient outcomes and operational efficiency. 

2. Program Development and Strategic Planning

Drive the strategic vision and growth of services with your dyad partner. Collaborate with IT and operations teams to ensure the integration and optimization of technology solutions to support the services provided. Develop and evaluate new models of care delivery, including expanding access to care for underserved and complex patient populations.  Look to expand geography of services provided.

3. Collaboration and Team Leadership

Work closely with interdisciplinary teams, including physicians, APPs, case managers, social workers, pharmacy, behavioral health, other health professionals, and IT professionals to deliver coordinated, patient-centered care.  Mentor and support clinicians providing care services, offering guidance on clinical decision making and practice management. 

4. Patient Centered Care

Ensure that patient satisfaction for services is up to Henry Ford standards. Monitor patient feedback and use data to drive improvements in care delivery and patient experience. 

5. Regulatory Compliance and Risk Management

Ensure compliance with healthcare regulations, including telemedicine guidelines, home health regulations, and privacy laws (e.g. HIPAA). Oversee risk management strategies, addressing potential issues related to patient safety, quality of care, and technology. 

6. Data Analysis and Reporting

Utilize data analytics to monitor and improve the performance of virtual care services and home-based care programs.  Prepare and present reports to executive leadership, summarizing key metrics, clinical outcomes, and areas for improvement. 

7. Clinical Practice

There would be an expectation to maintain some clinical presence in one or more of the services listed above.

Qualifications:

MD or DO with board certification in Internal Medicine, Family Medicine, or Geriatric Medicine. Relevant clinical experience, including experience with virtual care platforms, telemedicine, and home-based care models. Experience in leading or managing complex care programs is strongly preferred. Strong understanding of healthcare technology, virtual care platforms, and remote patient monitoring tools. Excellent communication, leadership, and collaboration skills.  Knowledge of healthcare regulations and compliance standards (e.g. HIPAA, telemedicine laws). Ability to analyze and interpret data to drive decision making and continuous improvement.

 

Preferred Qualifications:

Advanced degree (e.g. MPH, MBA, or MHA). Experience with quality improvement initiatives and value-based care models. Prior experience in a leadership role overseeing virtual or home-based programs.
