Summary:
The Medical Discharge Planner will be responsible for developing and facilitating discharge plans for all incarcerated patients at the Rhode Island Department of Corrections (RIDOC) who require a medical residential placement upon release or other specialized post-release medical care to be determined in coordination with the RIDOC medical team. This position will be primarily based at the RIDOC campus in Cranston RI.
Responsibilities:
Takes referrals from RIDOC�s medical team transitional services staff and parole board; reviews patient medical records and conducts patient interviews to determine appropriate post-release placement.
Assists with benefits enrollment.
Establishes and maintains a network of contacts at skilled nursing assisted living hospice and other facilities to facilitate patient placement.
Develops specialized medical discharge plans and prepares patients for medical release.
Conducts post-release follow-up to ensure fulfillment of discharge plans and evaluate placement success.
Submits logs and/or other Lifespan and RIDOC documentation of activities as requested for entry into a computerized system to track necessary data regarding client compliance and outcomes.
Performs other related duties as required.

BASIC KNOWLEDGE:
Bachelor� degree in Nursing with current license to practice as a Registered Nurse in the State of Rhode Island or in the nurse�s primary state of legal residency if that state is a member of the Nurse Licensure Compact OR Licensed Practice Nurse (LPN) OR Bachelor�s degree with relevant training and experience.
Center Case Management (CCM) � board certification or Accredited Case Manager (ACM �) is desirable.
EXPERIENCE:
Five (5) years of clinical experience that includes recent experience with care management patient navigation case management or discharge planning is strongly preferred.
