Medical Laboratory Sciences Knowledge Program Application
The Baptist Health Louisville Knowledge program has helped numerous individuals reach their career goals of becoming a professional in healthcare. The Knowledge Program is open to both current employees and external candidates and provides tuition funds in the form of loan forgiveness to individuals who have been accepted into a MT or MLT program. Upon graduation students will be asked to join Baptist Health and begin their Laboratory career.

Program Requirements
The Medical Laboratory Sciences Knowledge Program requires individuals to:
Be accepted and/or actively enrolled in a MT or MLT program.Hold a GPA of 3.0 or greaterProvide two letters of reference from school instructors or employersProvide a one page document outlining how your funds will be used, why you want to pursue a career at Baptist, and your post-graduate career plans, etc..After completing your application, related documents should be forwarded to Human Resources:

Program Commitments
Knowledge Program money is generally used for tuition but may be used for expenses at the recipient’s personal discretion. Program participants will be asked to sign an agreement prior to disbursement of Knowledge dollars.
Participants in the program must agree to work for Baptist upon graduation from their program. For every $2,500 in Knowledge Dollars the graduate will be required to work 6 months full-time to forgive the loan.
Knowledge Program participants are taxed according to IRS guidelines regarding Loan Forgiveness. Once the participant begins work as a Laboratory professional the loan monies are taxed through installments in the payroll process.
