Healogics is the largest provider of advanced wound care services in the United States, treating more than 300,000 chronic wound patients annually across over 600 sites. With an aging society, obesity and diabetes on the rise, and an uptick in surgical procedures, the number of patients with non-healing wounds that would benefit from expert care is dramatically increasing. As a result, the company is working to provide our differentiated, quality outcomes to as many patients that would benefit through our out-patient clinic partnerships.

The Medical Records Clerk is a member of the Healogics At Home Documentation Team for Durable Medical Equipment (DME) who works closely with outside partners and/or vendors and staff throughout the organization. The duties and responsibilities of the Medical Records Clerk is retrieval & management of medical records to help support our “Missing Documentation” reporting each day. Responsible for Record retrieval and uploads daily to our Documentation Repository System for DME documentation processing/auditing and compliance with state and federal regulations as well as HIPAA. The DME Medical Record Clerk, contributes directly and indirectly to the Healogics Key Results of Grow the Business and Exceptional Patient Experience.
The Medical Records Clerk will understand and fully support the mission, vision and value statements of Healogics, LLC.

All Healogics employees must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable.

Essential Functions/Responsibilities: 

Gather patient Medical Records/Documentation from i-Heal (Wound Assessments, Procedure Notes & Progress Notes)

Upload and maintain Documentation Repository System with medical records to our documentation vendor/partner according to laws and regulations daily

Requests Medical Records from other EMR systems outside of i-Heal, when applicable

Assists in the collection of missing documentation per the request of our vendor/partner

Help with departmental audits and vendor audits by requesting, retrieving, and uploading requested documentation

Ensure all medical records are protected/encrypted and kept confidential

Record and track all records requests (Documentation Repository System)

Completes clerical duties including answering phones, taking messages, responding to emails, communicating information back to partner/vendor, maintaining medical record logs and EMR systems and collaborating with fellow team members

Participate in improving quality throughout the organization

Performs other duties as required

Required Education, Experience and Credentials:

High school diploma or equivalent qualification

A minimum of 2+ years’ experience in a similar role in a healthcare setting

Advanced understanding of medical terminology and administration processes

DME experience preferred but not required

Required Knowledge, Skills and Abilities:

Comfortable in collaborating with healthcare professionals

Strong organizational and office technology skills

Knowledge of HIPPA and client confidentiality laws

Working knowledge of DME preferred but not required

Advanced understanding of medical terminology and administration processes

Proficient in information management programs and MS Office (Excel, Word, PowerPoint, Outlook)

Outstanding communication and interpersonal abilities

Strong attention to detail with excellent organizational skills

Strong level of confidentiality due to sensitivity of materials and information handled.

Ability to work independently and be self-directed and flexible

Ability to prioritize

Ability to perform functions with minimal supervision

Ability to work at a high-volume level of accuracy
