Assembles, maintains, and audits records of newly admitted, transferred, discharged and existing residents per set criteria. Establishes and maintains indexed system of ordering and thinning of files to ensure accessibility and prompt retrieval. Regularly audits records and filing system. Able to work independently and organize workload; ability to communicate with physicians, nursing personnel and allied health personnel.   2. Maintains and monitors resident calendars in connection with monitoring frequency of physician visits, notification of physician(s) when due, maintenance of schedule for residents’ physical exams, immunization, TB screening, and comprehensive assessments.   3. Initiates, maintains, and monitors Medicare and Medicaid certifications.   4. Prepares and maintains admission packets per facility policy.   5. Maintains clinical record control system. Labels active records with pertinent “alert” information such as allergies, CPR status, and codes diagnoses for assessment and billing purposes.   6. Maintains security and controls release of information to authorized persons only upon approval of Executive Director and Corporate Legal.   7. Performs quality assessment and assurance functions, as directed.   8. Assists with development and update of policies and procedures to reflect the philosophy of the facility, professional standards, and legal requirements.   9. As directed, participates with Executive Director/designee and/or Director of Nursing/designee in the survey processes.   10. Maintains and applies current skills and knowledge through continuing education and in-service programs.   11. In an emergency, carries out assigned responsibilities to assure resident and employee safety.   12. Carries out responsibilities in compliance with federal, state, local laws and regulations, and with facility philosophy, policies and procedures.   13. This position requires the ability to read and interpret technical journals, financial reports, and legal documents; ability to respond to common inquiries or complaints from residents, regulatory agencies, or members of the business community; ability to present information to management effectively.   14. Must be able to speak, understand and read the English language to the extent necessary to safely and properly care for residents.    15. This position requires excellent interpersonal skills with high level of energy and enthusiasm; good verbal and written communication skills; and the ability to organize, document, and implement detailed programs.   16. This position requires standing, sitting, stooping, pushing, pulling, lifting up to 50 pounds or more, bending, climbing, twisting upper body, walking, running, carrying, listening, talking, use of telephone, and exposure to medical equipment and persons with risk of illness.   SUPERVISORY RESPONSIBILITIES: None.   EDUCATION and/or EXPERIENCE: 1  + years of experience preferred in Long Term Care setting, LPN experience preferred.   CERTIFICATES, LICENSES - None preferred   REGISTRATIONS: None required. Driver’s license preferred.
