JOB SUMMARY:
The Medical Respite Assistant is a member of the integrated health care team that supports high quality and safe recuperative care and respite goals 24 hours a day in a Medical Respite Center. The Medical Respite Assistant participates in the implementation of treatment plans and daily respite activities consistent with preventative and holistic care, promoting a strengths-based, wellness-informed and person-centered approach to assisting individuals in achieving wellness and self-management goals. The Medical Respite Assistant supports the overarching mission to champion health and provide quality health services for at-risk residents. The Medical Respite Assistant reports directly to the Medical Respite Program Manager or Designee.
Collaborate with the Program Manager and treatment team throughout each shift to determine work assignments and priorities, as well as communicating the status of delegated task completion and outcome of patient data findings Collect and document data, including vital signs and other assessments as assigned per shift Assist patients with activities of daily living, including self-care, meal preparation, ordering and obtaining personal supplies, recreation, medication administration, wellness activities, and documentation according to individual treatment plans and, for overnight shifts, following program procedures for patients Monitor respite patients, including hourly bed checks and documentation, and provide close observation as needed Share observations, interactions and knowledge to develop 24 hour care plans; maintain knowledge of patient treatment and care plans Communicate any pertinent information at change of shift, including appointments, patient behaviors, changes in medications, etc. Coordinate safe and timely transportation for patients to off-site appointments as assigned Treat each patient in a manner that preserves and supports patient confidentiality, patient rights, and patient dignity Provide phone coverage and triage of calls when necessary, particularly during evening and weekend coverage Support a safe and clean environment of care, including assisting with daily cleaning and patient laundry Support meal time needs and cleaning up after the meals are finished Support others in learning how to make good choices for themselves, to become empowered and self-responsible Assist in crisis situations as appropriate and follow emergency protocols and procedures Assist the treatment team in referrals and coordination of care Participate in integrated team clinical case reviews Support collaboration with internal and external providers and resources to assist patients in reaching their goals Support the patient and care team with private and public assistance programs and resources of benefits and basic needs including transportation, housing, food, clothing, legal, vocational, and health services Promote patient engagement in treatment and therapy services as well as program and respite activities Exhibit enthusiasm, respect, adaptability, flexibility and spirit of cooperation in the integrated environment of care Maintain professional standards and provide timely and appropriate follow up for patients, including phone calls/text, emails, and written communication Ensure timely and accurate documentation of services provided within the Electronic Health Record and other documentation tools Check inventories on a weekly basis, report needs and stock supplies Maintain compliance with HIPAA and Confidentiality standards Adhere with organizational policies and procedures Participate in annual reviews, competencies, and other requirements consistent with organizational standards Participate in staff meetings, trainings, and Assist in other duties and projects as assigned
REQUIREMENTS:
Maryland Driver’s license preferred Basic computer skills and typing Strong organizational skills and attention to detail Strong interpersonal communication and relationship building skills Superior written and verbal communication skills Ability to work with various cultural and diverse individuals Bilingual English/Spanish a plus CPR certification through the American Heart Association required prior to start date ServSafe Basic Food Handling Certification preferred
