We are seeking a dedicated Medical Social Worker (MSW) to join our Covenant Home Health team on a PRN basis. In this role, you will collaborate with the physician and other healthcare professionals to address the social and emotional aspects of patients' health concerns. You will play an integral part in developing individualized care plans, documenting clinical progress, supporting families, and connecting patients with community resources.
Additional responsibilities include participating in discharge planning, in-service programs, and offering expert consultation to Covenant Home Health Agency staff.

Responsibilities Understands and adheres to Agency policies/procedures. Assists the physician and other team members in understanding the significant social and emotional factors related to the patient’s health problems. Participates in the development of the patient’s plan of care as requested by the Director of Nursing. Conducts educational groups related to aging and geriatric population including Alzheimer’s and Dementia education. Completes clinical documents and progress notes for the patient’s electronic medical record in a timely manner. Assists the client and/or POA with appropriate community resources. Participates in Agency Care Conferences and QAPI program. Acts as a consultant to other Agency personnel. Provide intensive and/or continuing counseling to patients in evidence of crisis or need. Observes confidentiality and safeguards all patient information. Serves as a resource person to employees, patients, physicians and other allied healthcare providers. Maintains contact with community support groups and provides professional expertise as required. Qualifications A Master of Social Work (MSW) degree from a program accredited by the Council on Social Work Education; orA Master of Social Work (MSW) degree from a program accredited by the Council on Social Work Education; orA Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a Baccalaureate degree in psychology, sociology, or another related field, with at least two years of social work experience in a healthcare setting, and under the supervision of a Home Health MSW.A minimum of One (1) year experience in Home Health preferred.Ability to exercise initiative and independent judgement.Skill in collaborating with individuals to gain the cooperation of many in order to accomplish a common goal.Benefits:
