We are currently seeking a passionate and dedicated Per Diem Medical Social Worker (MSW) to provide support and care in the comfort of patients' homes.
Location: Bend, ORSalary: Starting at $38/ hour, DOE!
Qualifications:- Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Master’s or doctoral degree in social work is a plus.- Licensed as a licensed clinical social worker.- Minimum of one (1) year of experience in healthcare, with preference given to candidates with experience in home health care.- Excellent verbal and written communication skills, as well as strong organizational abilities.- Current CPR certification.- Must be a licensed driver with access to a reliable, insured vehicle in good working order.
Responsibilities:- Assess the psychosocial status of patients in relation to their illness and environment.- Conduct social evaluations and develop intervention plans based on assessment findings.- Maintain accurate clinical records for all patients referred to social work services.- Provide information and referral services to patients and families/caregivers regarding practical and environmental needs.- Serve as a liaison between patients/families/caregivers and community agencies.- If you are ready to contribute to our mission of providing exceptional care and support to those in need, apply now
