**Responsibilities:**
+ Work directly with Members, pharmacies, and Providers to assess medication adherence patterns, identify factors contributing to non-adherence, and support the medication adherence program;
+ Coordinate adherence strategies to meet Member needs, including transitioning Members to 100-day & mail order prescriptions
+ Sync medications to refill at the same time
+ Provide refill reminders for Members and caregivers
+ Educate Members on the importance of medications
+ Collaborate with Members and pharmacies to ensure medications are picked up and taken as directed;
+ Engage with Providers to coordinate prescription refills and renewals
+ Engage with pharmacies to coordinate prescription fulfillment
+ Proactively identify Members who may be at risk for medication non-adherence
+ Serve as a Care Team resource for medication-related inquiries pertaining to formulary tiers, prior authorizations, formulary exceptions, and cost-sharing
+ Document Member care activities in the Electronic Medical Record (EMR) to track clinical pharmacy interventions and oversight responsibilities;
+ Reconnect Members with the Primary Care Team, if necessary, to help coordinate the completion of their quality health maintenance
+ Work efficiently with multiple Providers and provide remote coverage of other markets as assigned;
+ Continuously learn and adapt workflows to improve Member care;
+ Assist with other Member care and quality initiatives as needed to promote positive health outcomes and support our healthcare team
+ Other duties as assigned by the Quality Pharmacy Manager
**Education/Experience:;**
+ High School Diploma or equivalent required
+ Associate’s Degree preferred
+ Active/current National Pharmacy Technician Certification (NHA or PTCB) **required**
+ **Requires:**
+ Minimum **three (3) years** experience in Medicare Part D pharmacy and/or Quality/STARS Measures and/or a managed care pharmacy setting
+ Current/recent relevant experience at an appropriate level of independence, complexity, and authority in managed care or healthcare environment
+ Proficient in Microsoft Excel
+ Proficient in Tableau preferred
