Medication Care Coordinator

Position Summary

Prepare prescribed medication(s) for outpatient use, including entering patient demographics into the pharmacy software system, labeling medications, and providing the prepared medication(s) to the prescribing provider for final review prior to dispensing while always maintaining accuracy and safety for Sanita's patients.

Medication Dispensing Support

Receive written/electronic prescriptions from providers and prepare the medication as prescribed by the provider Process and maintain medication order queue in an optimal time frame, communicate with your supervisor if orders are falling behind and you have more than 3 patients waiting on medications Process all EHR Pharmacy related messages and faxes on the same business day. Ensure all medication prior authorization requests are reviewed and worked on daily Communicate any EHR task or message that needs to be addressed by your supervisor in a timely manner Process reverse medication requests within 12 hours of receipt Email your supervisor all the necessary information regarding reverse medication requests within 12 hours from the time the medication was reversed Confirm the information on the prepared medication is complete, accurate, and matches what

was prescribed by the provider

Enter patient demographics into the pharmacy software system as needed, ensuring accuracy of information entered Provide prepared medication(s) to the corresponding provider for verification, signature, and dispensing Ensure dispensing log is complete and accurate within 24 business hours, notify the supervisor if unable to meet this deadline Notify provider team of any prescription denials to help them determine a course of action and your supervisor for documentation purposes Review failed electronic submitted prescriptions and re-submit as needed, notifying immediate supervisor of repetitive issues/patterns Identify backup provider to sign prescription in another providers absence Print medication inventory par levels before the 8 AM every Monday or at the beginning of each work week (or as frequently as directed by a supervisor) Mark medications within 120 days of expiration with approved identifier and notify supervisor Log expired medications in the Expired Medication Log Place all expired medications in the expired bin and notify supervisor to coordinate witnessed disposal Receive and organize medications according to the medication type, lot number, and expiration date (rotate stock as needed with more recent expiration date(s) moved to front of bin/shelf) Assure that two employees verify all medications received Keep an updated inventory of the medication available in the dispensing room and system Organize and adjust inventory for any medications received from a transferring clinic that may be approaching expiration date and label them accordingly (rotate stock as needed with more recent expiration date(s) moved to front of bin/shelf). Maintain transfer record log and file transfer documents accordingly Inform the supervisor and providers of any medication inventory shortages/unavailability Inform any discrepancy identified in the pharmacy platform to the supervisor and keep the system updated where applicable and within scope Report all medication related incidents (i.e., wrong sig dispensed, wrong medication, wrong patient, etc.) to the supervisor for tracking purposes and safety plan of action Ensure the security and proper storage of medications is maintained at all times Perform general clerical duties, such as answering phones, processing payments, among others Process patient medication requests in accordance with company policy Maintain patient confidentiality; complies with HIPAA and compliance guidelines established by SANITAS USA Maintain detailed knowledge of computer software as it relates to job functions Attend CPR, HIPAA, and OSHA training programs as required Comply and complete all mandatory compliance training provided by SANITAS USA and its affiliate companies Maintain certifications and registrations per state guidelines Attend all regularly scheduled meetings Perform all other tasks and projects assigned by the Director of Pharmacy, supervisor, and/or provider Maintain a clean and orderly work environment Supervisory Responsibilities

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. The employee is subject to environmental conditions, protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level. Position requires full range of body motion, manual and finger dexterity, and eye-hand coordination. Employee may work under stressful conditions and work irregular hours. Employee will be exposed to biohazard materials on a regular basis. Employee will have some exposure to communicable diseases, toxic substances (ie: cleaning products), medicinal preparations, and other conditions common to a clinic environment.
