Medication Collection Technician
Job Summary Under the general direction of the hospital pharmacy, Medication Collection Pharmacy Technicians will independently perform tasks related to collecting, reconciling, and entering the home medication history into the patient's electronic medical record. Information will be obtained by interviewing the patient, patient family, calling local pharmacies and using other available resources for the purpose of compiling a medication history. Compiled medication history information is entered into the patient's electronic health record for reconciliation by the admitting provider and will be used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. Medication Collection Pharmacy Technicians interact with and provide medication history and other related patient information to nurses, physicians, pharmacists, and other medical professionals as appropriate. This position will have an impact on improving safety and quality of our patients by providing physicians and physician extenders with a comprehensive home medication list. May perform a variety of functions which impact the medication reconciliation process and patient care. A Medication Collection Pharmacy Technician also assists the pharmacist in his/her professional responsibilities in providing pharmacy services.
